Vantage Holding Company, LLC, has received a notification of a privacy incident

Vantage Holding Company, LLC ("Vantage") is giving notice of a recent incident that may affect the security of certain information relating to individuals. Please note that Vantage has no knowledge of the cause of this incident.

On November 18, 2021, Vantage discovered that certain computer systems were accessed and that limited information, which may have included personal information, was taken by an unknown actor.

Vantage has determined through a research that the information at danger includes certain individuals' names, addresses, dates of birth, and health information, including diagnostics or conditions, lab results, and medications.

Vantage began giving written notices to people affected by the incident on January 14, 2022, and teaches them how to protect their information. In addition, Vantage provides information on identification theft and fraud alerts, examining their accounts, and monitoring their credit reports for suspicious activities.

As part of Vantage's ongoing commitment to information privacy, Vantage has launched a deep investigation to see the scope of the incident. Additionally, Vantage is also considering adding additional steps to help protect against the same type of incidents in the future.

Vantage has established a dedicated call center for individuals to contact with questions or concerns or to determine if they are affected. The dedicated call center can be reached at 855-604-1764 Monday through Friday, from 9:00 a.m. to 9:00 a.m. Eastern Time.
